Purpose of this form: To record complaints, feedback, rumors and suggestions from project participants, staffs, and other stakeholders. Feedback can be raised through any channel a community member or person deems available or preferable to them. These include during routine field activities, meetings, walk-ins to the office, or calling the dedicated line (hotline) and emails. Each staff in field sites should carry and fill a blank copy of this form with them each day they spend in field locations. The form should be submitted to MEAL Office for entry into the Feedback database/log. DO NOT complete Background Info if a participant chooses to remain anonymous as the record of their personal information will be unavailable. Please record major issues that you hear from communities each day. If there are any issues related to protection, corruption or abuse, please report these immediately to your supervisor.
